Hickey (eds.), Controlled Pulmonary Drug Delivery, Advances in Delivery Science and Technology, DOI 10.1007/978-1-4419-9745-6_2, © Controlled Release Society 2011 Abstract Delivering therapeutic agents to the lungs requires a deep understanding of the kinetics and dynamics of drugs in this biologically and physiologically complex system. In this chapter these concepts are discussed and include drug dissolution rates in the airways, physical clearance mechanisms of the mucociliary escalator and cough, alveolar macrophage clearance, pulmonary metabolism, and pulmonary absorption. Finally, these aspects are considered together with drug and formulation aspects as determinants of duration of effects of inhaled products. The mechanisms of elimination of drug activity in the lungs by the various clearance processes described here are important factors to consider both in the development of new drugs and in understanding the relative merits of existing therapies.

Inhaled nano- and microparticles for drug delivery
The 21st century has seen a paradigm shift to inhaled therapy, for both systemic and local drug delivery, due to the lung's favourable properties of a large surface area and high permeability. Pulmonary drug delivery possesses many advantages, including non-invasive route of administration, low metabolic activity, control environment for systemic absorption and avoids first bypass metabolism. H...
متن کاملContribution of rat pulmonary metabolism to the elimination of lidocaine, midazolam, and nifedipine.
The contribution of the lung to drug metabolism was investigated in rats and the possibility of prediction of in vivo metabolism from in vitro studies using rat pulmonary microsomes was assessed. Lidocaine, midazolam, or nifedipine was administered to rats at a dose of 10 mg/kg by the intra-arterial, intravenous, and intraportal routes. The pulmonary extraction ratios of lidocaine, midazolam, a...

The nasal cavity possesses many advantages as a site for drug delivery, such as, ease of administration, applicability for long term treatments and a large surface area for absorption. One important limiting factor for nasal drug delivery is the limited time available for absorption within the nasal cavity due to mucociliary clearance. Several drug delivery systems including different kinds of ...

متن کاملPopulation pharmacokinetics of rifampin in pulmonary tuberculosis patients, including a semimechanistic model to describe variable absorption.
This article describes the population pharmacokinetics of rifampin in South African pulmonary tuberculosis patients. Three datasets containing 2,913 rifampin plasma concentration-time data points, collected from 261 South African pulmonary tuberculosis patients aged 18 to 72 years and weighing 28.5 to 85.5 kg and receiving regular daily treatment that included administration of rifampin (450 to...
